Effective April 7, 2017 Diabetes America has ceased operations in the following clinics:

  • Medical Center Houston
  • Katy
  • Sugar Land
  • Pasadena
  • Humble
  • CyFair
  • Plano
  • Arlington
  • South Alamo Medical Clinic

Between April 10, 2017 and May 5, 2017, the following Diabetes America locations will remain open for patient care:

  • Pearland
  • Central San Antonio
  • Las Colinas

After May 5, 2017, all Diabetes America locations will be closed permanently.

Additionally, until you’ve chosen a new diabetes provider, for your medical care you may reach out to:

  • the Dallas, Harris or Bexar County Medical Society for a list of diabetes providers
  • your insurance carrier for an approved list of providers 
  • your local urgent care clinic
  • UT Physicians 24/7 Nurse Triage Line 844-880-4103
  • for any medical crisis or exacerbations, please immediately go to the nearest Emergency Room.

You may arrange for the transfer of your medical records to your new Provider once you’ve chosen one. Please notify the medical records custodian, UT Physicians, at 888-4UT-DOCS (888-488-3627) or go to to obtain forms for transfer of care, medical records release, or for any questions about your medical records.

Curant Health, our pharmacy partner, is committed to continuing to provide outstanding medication management support for patients:

  • If you are a Curant Health patient today, your prescription refills for the next 90 to 180 days have already been sent to Curant Health.
  • If you are not a Curant Health patient today, Curant Health staff will be on site at the remaining open clinics to answer your questions about personalized medication management during your upcoming appointment OR you can visit to learn more about their program.
